Interview anxiety destroys me (even though I'm qualified)

Phone screen: mind goes blank. In-person: can't make eye contact, ramble, forget what I wanted to say. Assessment tests: fail even when I'd pass on the job. How do I get past interviews?

Competence doesn't translate to interview performance.
